On the 16th anniversary of Teoh Beng Hock's mysterious death, MACC today has issued its first public apology to the political aide's family for their pain and suffering.
In a statement, MACC chief commissioner Azam Baki also announced the anti-graft agency's readiness to provide a "goodwill contribution" to Teoh's family, particularly to support his only son's welfare and educational needs.
"MACC would like to take this opportunity to acknowledge the findings from various investigations over the past 16 years regarding the death of Beng Hock.
"These include the royal commission of inquiry (RCI) in 2011, the Court of Appeal’s decision in 2014, and the most recent investigation by the police, which was classified as no further action.
"Although the latest investigation did not uncover sufficient evidence to charge any individual, the MACC views with utmost seriousness the fact that Beng Hock was found deceased on Selangor MACC premises on July 16, 2009,” he said.

Azam added that the tragic incident had a profound impact not only on the deceased’s family but also on all MACC personnel and the nation.
"In light of this, as the MACC chief commissioner, I wish to express my deepest sympathies and extend an apology to the Teoh family for the pain and suffering they have endured over the years," he said.
Enhanced safety measures
Azam also pointed out the steps MACC had taken to address its shortcomings and safeguard the safety of individuals summoned for investigation.
These include establishing video interviewing rooms for recording of statements, setting up interview rooms on the ground floor, and fitting MACC buildings with iron grilles.

MACC had also increased and upgraded its CCTV systems with advanced technology, and equipped investigating officers with body cameras to record all operations.
While the government had paid RM600,000 in compensation to Teoh's family and another RM60,000 in legal costs, Azam said MACC is prepared to provide the family with a contribution to help support the deceased's child.
"MACC remains steadfast in its commitment to uphold integrity and transparency in all aspects of its duties and to continuously strengthen institutional reforms," he added.
In 2009, Beng Hock, an aide to then-Selangor state executive councillor Ean Yong Hian Wah, was found dead after overnight questioning at Selangor MACC, which was then headquartered at Plaza Masalam in Shah Alam.

A 2011 RCI ruled that he was driven to suicide due to aggressive interrogation.
However, in 2014, the Court of Appeal ruled his death was caused or accelerated by unlawful acts, including those of MACC officers.
Last year, Prime Minister Anwar Ibrahim pledged to the family that the case would be investigated thoroughly.
However, in May, the Attorney-General’s Chambers ultimately classified the case as NFA.
